Below is a quick guide to becoming a Medical Cannabis Patient in the State of Missouri.
Step 1
Talk to your doctor and see if you have a qualifying condition per Article XIV of the Missouri Constitution. We have made a handy list of the current list of qualifying medical conditions for getting a medical cannabis card in Missouri. DHSS Would Like To Remind The Public That Any Physician Certifications They Receive For Medical Marijuana Must Be No More Than Thirty Days Old At The Time An Individual
Applies For A Patient Identification Card, And DHSS Will Not Begin Accepting Patient Identification Card Applications Until July 4, 2019.
Missouri residents must see a state-approved medical marijuana physician, either physically or virtually, to obtain a certification for a qualifying condition. This certification is used to submit with your application for your patient card.

- Cancer
- Epilepsy
- Glaucoma
- Intractable migraines unresponsive to other treatment
- A chronic medical condition that causes severe, persistent pain or persistent muscle spasms, including but not limited to those associated with multiple sclerosis, seizures, - Parkinson’s disease, and Tourette’s syndrome
- Debilitating psychiatric disorders, including, but not limited to, post-traumatic stress order, if diagnosed by a state licensed psychiatrist
- Human immunodeficiency virus or acquired immune deficiency syndrome
- A chronic medical condition that is normally treated with a prescription medications that could lead to physical or psychological dependence, when a physician determines that medical use of marijuana could be effective in treating that condition and would serve as a safer alternative to the prescription medication
- A terminal illness
- In the professional judgment of a physician, any other chronic, debilitating or other medical condition, including, but not limited to, hepatitis C,
- amyotrophic lateral sclerosis, inflammatory bowel disease, Crohn’s disease, Huntington’s disease, autism, neuropathies, sickle cell anemia, agitation of Alzheimer’s disease, cachexia, and wasting syndrome.

To complete your patient application online, you will need the following:

Mailing Address or place where a qualifying patient can receive mail
Additional Information
At the option of the applicant, a statement indicating whether the qualifying patient is currently receiving assistance from any Missouri programs for low-income individuals, and if so, which programs.
If the patient is seeking authority to cultivate medical marijuana.
Attestation statement; Signature and date of the application; All applicable fees.
Proof of Missouri Residency (one of the following)
Copy of a valid Missouri Driver’s License
Missouri Identification Card
Current Missouri Motor Vehicle Registration
Recent Missouri Utility Bill
Physician Certification Form
As part of the application process, Patients must obtain a Physician Certification that is issued no later than thirty days prior to the time the patient submits their application.
A Missouri-licensed physician, active and in good standing, is required to complete and sign the certification.
Physicians will submit the form electronically through the application website. Patients will attach the form through their application when applying.
Additional Required Items
A legible copy of the qualifying patient’s photo identification issued by a state or federal government entity
A Parental/Legal Guardian Consent Form for non-emancipated qualifying patient.
A clear, color photo of the qualifying patient’s face taken within the prior three (3) months.
Caregiver?
Medical Cannabis Program patients can designate one caregiver to apply for the caregiver card. The caregiver card will allow the person to legally purchase and transport the medicine to the patient. Caregivers need the same required documents the patient needs excluding the doctor form. The fee for a caregiver is $25 a year
Minor qualifying patients need a physician certification AND a reviewing physician certification. A designated caregiver must apply, providing all the same documents listed for a patient.
Opioid patients can NOT have a caregiver.
Forfeitures?
Medical Cannabis card holders DO NOT have to forfeit their FOID or Concealed Carry Card.

On June 28, 2019, the Missouri Department of Health and Senior Services began taking applications from qualifying patients and caregivers for medical marijuana ID cards. Patients and caregivers must be state residents to apply. The patient/caregiver registration website is available on medicalmarijuana.mo.gov. The state health department only accepts applications submitted through the online registry.
After uploading an application, patients will receive an electronic notification about the status of their application, frequently within 48 hours. The health department does not provide physical cards to patients. Instead, patients should log into their Missouri Medical Marijuana Program Registry account to download a copy of their card, then print it out.

No. There is no reciprocity between Missouri’s Medical Marijuana Program and other state medical marijuana programs. Just because you are licensed in another state doesn’t automatically qualify you for a license in Missouri. Patients may purchase medical marijuana in Missouri only upon presentation of a Department-issued patient identification card.

Per 19 CSR 30-95.030, no qualifying patient shall consume marijuana for medical use in a public place, unless provided by law. 19 CSR 30-95.010 defines a public place as any public or private property, or portion of public or private property, that is open to the general public, including but not limited to, sidewalks, streets, bridges, parks, schools and businesses. See 19 CSR 30-95.010(33) for more information.

Patients and caregivers with a valid department-issued identification card may legally possess medical marijuana now subject to the requirements and limitations in 19 CSR 30-95.030 and Article XIV of the Missouri Constitution.

The qualifying patient’s physician may certify up to four ounces of dried, unprocessed marijuana, or its equivalent, in a 30-day period. If there is a compelling reason why the qualifying patient needs a greater amount, then the Department requires two independent physician certifications specifying what other amount the qualifying patient needs. The patient identification card does not offer individuals protections from violating laws pertaining to operating a motorized vehicle while under the influence. Nothing in Article XIV permits a person to operate, navigate, or be in actual physical control of any dangerous device or motor vehicle, aircraft or motorboat while under the influence of marijuana.
